It can be considered as a simplified intervention of Duodenal crossing providing the benefits of a bilio-pancreatic shift, the pyloric preservation and a simpler operation.

The published good short and medium term results, as for loosing weight and the resolution of comorbilities indicate that SADI can be an effective option as second surgical stage after a Sleeve gastrectomy.

It is recommended for adult patients (18 to 65 years old) affected by Obesity type IV (above all in IMC > 55 kg/m2), jointly with a change of the style of life (diet and exercise).

As in the Gastric By-pass, losing weight is achieved by means of two mechanisms, matching a vertical gastrectomy (restrictive component), jointly with the small bowel crossing, in order that it only used 50% thereof for the digestion and absorption of the foods (malabsorptive component).

The procedure is carried out by means of laparoscopy, and is associated to a systematic cholecystectomy. Because of the complexity of the technique, and with the objective of reducing the surgical risk, a two-stage surgery may be proposed (first a vertical gastrectomy and, in a second stage, the bilio-pancreatic shift) or the possibility also exists to carry out an intervention with a sole duodeno-ileal anastomosis (SADI), with promising results. As a significant malabsorptive component is present, it requires a strict control by an endocrinologist and a nutritionist, to prevent possible nutritional deficiencies.

The Bilio-pancreatic shift with Duodenal Crossing (Switch) achieves an over 75% lost of overweight after 5 years, jointly with the improvement o even remission of the comorbilities associated to the Obesity, such as high blood pressure, diabetes mellitus type 2, a dyslipaedemia and the sleep apnoea syndrome.

It is carried out in adult patients (18 to 65 years old) with Obesity type III (above all in IMC > 45 kg/m2) or Obesity type II associated to diabetes mellitus type 2, who failed when trying to losing weight with diet and exercise.

Loosing weight is achieved by means of two mechanisms, matching a very reduced stomach capacity (restrictive component) jointly with crossing the small bowel (By-pass) entailing that only 60% thereof is used for the absorption of nutrients (malabsorptive component).

The Gastric By-pass achieves loosing 60-70% overweight after 5 years, jointly with the improvement or even remission of the comorbilities associated to the Obesity, such as high blood pressure, diabetes mellitus typo 2, dyslipidaemia and sleep apnoea syndrome.

It is indicated in 18 to 65 years old patients having Obesity type III (IMC > 40 kg/m2) or Obesity type II with greater comorbilities (high blood pressure, diabetes mellitus type 2, dyslipidaemia, sleep apnoea syndrome) who do not achieve losing weight despite they followed diets and indications of changing their style of life.

The Vertical Gastrectomy achieves a loss of overweight of 50 to 60% after 5 years, jointly with an improvement or even a remission of the comorbilities associated to the obesity, such as high blood pressure, diabetes mellitus type 2, dyslipidaemia and sleep apnoea syndrome, and without important substituting requirements from the nutritional and endocrinological point of view.

The great value of the Intragastric Balloon or Gastric Balloon is that it helps to regulate the intake amount, preventing eating too much and facilitating to follow a re-education of the nutritional habits and of the style of life, with the objective to acquire healthy dietetic and life habits that, in addition to achieve losing weight, facilitate keeping the lower weight after withdrawing the balloon.

The Endoscopic repair of a Gastric By-pass by means of a System of Endoscopic Suture OverStitch of Apollo Endosurgery, consists in undertaking a reduction by endoscopic route of the dilated gastro-jejunal anastomosis, without having to have recourse to external incisions.

Through an endoscope introduced through the mouth, sutures are made in the gastro-jejunal anastomosis to reduce its size, achieving a slower passage of the food, entailing an early satiety

and delaying the appearance of hunger feeling.

By means of this endoscopic suture system the stump or gastric pouch can be reduced in the event of an overdilatation.
